In this webinar we will analyse the human causes of errors leading to serious incidents, focusing on unintended errors (slips, lapses, and mistakes) and intended errors (workarounds) in different control situations. We will emphasise the need for organisations to improve control enablement and worker engagement to reduce these errors

Key Learnings:

  • Differentiate the causes and contexts of human errors: distinguish between the three main categories of human error - slips/lapses, mistakes, and intended workarounds.
  • Analyse the cognitive factors driving unintended errors and propose mitigation strategies.
  • Evaluate organisational reliance on human performance and articulate the need for system resilience.
